After-School Qur’an Program — Payment Consent & Liability Waiver

By selecting “I Agree” and clicking Submit, I acknowledge that my selection constitutes my electronic signature and consent to the terms below:

1. Payment Authorization
   I authorize Quran for Life Institute (QFLI) to process tuition payment(s) for the selected class. If I choose Stripe Auto-Pay, I understand I can cancel auto-pay at any time before the next billing cycle.
2. Refund Policy
   The registration/tuition fee is refundable within 24 hours after the first session of the term. After this window, payments are non-refundable.
3. Minimum Enrollment
   Classes require a minimum number of students. If the minimum is not met, the class may be canceled and payments will be fully refunded.
4. Late Fee
   A 10% late fee may apply to payments made after the 28th of the month for the upcoming month’s enrollment.
5. Attendance & Make-ups
   QFLI will reschedule any canceled session(s) to ensure the full monthly 8 hours of instruction are delivered.
6. Liability Waiver & Release
   I give permission for my child to participate in the selected class/activity and I waive and release Quran for Life Institute and Al-Hadi School, including their officers, directors, employees, agents, and leaders, from any and all liability for injuries or damages arising from participation in this class/activity, except where prohibited by law.
